Intake Specialist/Resident Reviewer (LPC or LSW)

The Resident Reviewer will serve as a member of the Assessment Team and will complete biopsychosocial assessments to individuals with severe and persistent mental illness within a long-term care facility. This position functions as a grant through the State of Illinois under the Colbert Consent Decree.

As the Resident Reviewer, you will be provided a laptop, office space, free clinical supervision, on-site clinical trainings, and mileage reimbursement for travel, conferences, and seminars.

Job Qualifications:

Master's Degree in a Social Service field and LPC or LSW; LCPC or LCSW preferred.

Must be willing to travel to various long-term care facilities throughout the Chicagoland area.

Must be able to work independently with minimal supervision.

Must be flexible to make changes as workload/demands change.

Minimum of one year's experience working with individuals with mental illness and working knowledge of the recovery model preferred.

Valid Illinois driver's license and documentation of current auto insurance, with a good driving record and car available.

Previous experience as an Intake Specialist or Resident Reviewer strongly preferred.

Position Responsibilities

Meet with members within the long-term care facility to complete biopsychosocial assessments.

Obtain collateral information from nursing home staff, family members, guardians, doctors, and other members involved in the member’s treatment.

Obtain necessary documentation such as face sheets, POS, and maintain tracking sheets to ensure accurate collection of all necessary documents from nursing facilities.

Complete six month reviews for members previously noted as recommended for transition.

Attend trainings off site as necessary to collaborate with the State regarding the Resident Review Process.

Complete full assessment and make recommendations regarding the transition of a member.

Complete mental health assessments for the Intake Department as assigned.

Other duties as needed/assigned.
